Brief Abstract Summary: Discover the barriers and challenges that children in foster care face regarding vaccination reconciliation. Discover how a standardized vaccination workflow can streamline the vaccination reconciliation process. Learn about evidence-based options to perform a quality improvement project based on improving feasibility and acceptability while including stakeholders.
Detailed abstract description: This poster presentation demonstrates actionable insights into developing a formal, streamlined vaccination protocol for vulnerable patients to create more equitable health processes. Attendees will hear about the evidence-based workflow, which can increase clinical efficiency, reduce time spent on vaccination reconciliation, and increase vaccination rates. It will demonstrate how to connect with multiple stakeholders to assess current workflows, barriers to individual processes, and comprehensive feedback to vaccination reconciliation. The participant will learn how quality improvement descriptive metrics were applied and evaluated. Lastly, the presentation highlights how stakeholder collaboration and clear vaccination reconciliation objectives help create meaningful changes for patients.
