Describe CDC’s NCCDPHP five SDOH domains and how STLT jurisdictions can incorporate PSE change into local activities. Describe four PSE change interventions implemented to address social drivers of chronic conditions.
Identify four strategies for engaging diverse community members and local partners in planning efforts to address SDOH.
Describe four PSE change interventions being implemented to address social or non-medical drivers of chronic conditions.
Brief Abstract Summary: Learn about the CDC’s National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) framework for social determinants of health (SDOH). The SDOH domains being addressed in the framework include the built environment, social connectedness, community-clinical linkages, food and nutrition security, and tobacco-free policy. Gain insight into community engagement strategies and tools used by state, tribal, local, and territorial (STLT) jurisdictions to develop implementation-ready plans focusing on the highlighted SDOH domains through the 1-year grant, "Closing the Gap with Social Determinants of Health (SDOH) Accelerator Plans”. Additionally, learn about policy, systems, and environmental change interventions proposed and implemented by communities to address non-medical drivers of health disparities and to prevent or reduce poor chronic disease outcomes among minority populations and those who are experiencing disadvantages and health inequities.
Detailed abstract description: The Improving Social Determinants of Health Act of 2021 authorized the CDC to establish a program aimed at enhancing health outcomes and mitigating health disparities. In 2021, the CDC's NCCDPHP initiated the SDOH Accelerator Plans with a 1-year grant named "Closing the Gap with the SDOH Accelerator Plans." This grant has completed 3 funding cycles providing support to 71 state, local, territorial, and tribal jurisdictions to develop Accelerator plans. These plans are designed to identify policy, systems, or environmental (PSE) change interventions to address SDOH domains associated with chronic disease outcomes and promote health equity within their communities. The presentation will offer an overview of the CDC’s NCCDPHP SDOH approach and domains of focus. The presenter will discuss effective community engagement strategies used for establishing multi-sector partnerships, conducting needs assessments, and strategic planning. Additionally, the presenter will highlight the process for identifying PSE interventions that can be implemented to address "social or non-medical drivers" that increase health disparities and prevent or reduce poor chronic disease outcomes among minority populations and those who are experiencing disadvantages and health inequities. The presenter will also share Accelerator plan interventions that are being implemented and key results.
